Instructions:

1. Fill out enrollment papers for each eligible individual member of your family, including children, grand-children, and great grand-children.

2. For every person submitting an enrollmet application whether its for yourself or a child, please provide the following items:

a. A certified copy of yours and your childs birth certificate. (Not the original)

b. For those people who are 18-years  and older, please attach a current photo (we recommend a passport picture which can be obtained at nearly any Post Office, Walgreens or Kinkos). The photo will be used for your Membership Identity Card. When processed the card will be sent to you.

c. Any incomplete applications will not be processed so if you have any questions, please call Arlene Gardner at (435) 789-7401.
